*About the Role:*
* Conduct home visits to assess patients and determine personalized care programs.
* Develop and implement individualized care plans, collaborating with our aides in the field.

*Requirements:*
* Registered Nurse (RN) with at least one year of nursing experience.
* Prior home care experience is preferred but not required.
* Strong interpersonal and communication skills.

Pay: $36.00 - $40.00 per hour

Expected hours: 30 - 40 per week
