WE ARE WILLING TO TRAIN.Multicultural Home Care, a Modivcare Personal Care Service, is looking for a Per Diem Nurse as a Field Supervisor responsible for ensuring quality and safe delivery of home health Care services such as supervising Caregivers/Certified Home Health Aides in a patient's home, evaluate the patient's plan of care and assess their physical and Psychosocial needs. Schedule: Flexible, will do home visits when aides in service. Performs on-site supervisory and teaching visits to evaluate caregivers Provides care orientation to caregivers on assigned home care cases. Participates in the training of new nurses and in-services training for field staff. Performs initial nursing assessments Developing a plan of care and makes necessary revisions based upon patient's condition/needs. Document notes ensuring any changes in the service or patient's condition are reported Monitors records for complete and accurate documentation. Coordinates visits and communicate changes with the teamWe are excited to speak to someone with the following Registered Nurse (RN) prefers, also will consider LPN.
