POSITION DESCRIPTIONTITLE: LPN (OSHA Classification 1)FLSA: Non-exemptPOSITION SUMMARY: Responsible for the overall operation and optimal quality of care for the residents on the assigned unit. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and Nursing policies and procedures.POSITION REQUIREMENTS - Education / ExperienceLicensed Practical Nurse licensed in the State of MassachusettsMinimum of one year of education or experience in geriatric nursing or rehabilitation preferred.CPR certified.Ability to read, write and comprehend English; ability to follow oral and written instructions.Fluent knowledge of the MS Office suite and other office equipment.Ability to work hours as scheduled based on the requirements of the position assignment.Working Conditions: Works in a well-lighted and ventilated work area.Physical Requirements:Must be able to move intermittently throughout the day, proper body mechanics required. Ability to perform physically demanding worked involving frequent bending, stooping, turning, stretching, and reaching above the shoulders are involved.Must be able to see and hear or use prosthetics/equipment that will enable these senses to function adequately to assure that the requirements of this position can be fully met.Must be able to lift a minimum of 50 lbs.BEHAVIORAL COMPETENCIESAccountabilityThe demonstrated ability for ownership of all actions and behaviors in fulfilling job responsibilities in a manner consistent with the organizations Mission and values. This competency is characterized by those actions and behaviors that contribute to creating an environment where all individuals rights are respected and upheld and are treated equitably and with dignity.TECHNICAL COMPETENCIESUnderstands and upholds the Residents Bill of Rights and holds all resident information confidential.Follow and comply with all State and Federal regulations and established Personnel Policies and Procedures.Maintain all pertinent records in a timely, efficient, and confidential manner.Project a professional image always.ESSENTIAL FUNCTIONSAdministers medication and treatments according to the physician's orders.Assumes responsibility for ordering medications.Transcribes, documents, and implements physician's orders.Collects data to assist in the nursing assessments during each shift.Gives daily shift report to CNAs assigned to the unit prior to the start of the shift.Reviews written daily assignments for each resident on assigned unit.Assures assignments are completed before the end of the shift.Gives and takes nursing report on assigned unit. Communicates essential information to appropriate parties.Makes rounds on assigned residents at least once every two hours.Collaborates with Care Plan Coordinator to develop and maintain nursing care plans and update these plans on a regular basis and as the resident's condition changes.Attends and participates in interdisciplinary resident care conferences as requested.Assists the attending physician with rounds and special procedures as needed.Participates in the yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps that need to be taken to improve their performance.Assists Staff Development with orientation and on-the-job training for new nursing personnel as needed.Collaborates with nursing supervisor in documenting disciplinary concerns for nursing assistants on designated shifts, including reprimands, oral and written warnings, and corrective action recommendations.Notifies nursing supervisor, residents physician, and family of a change in the residents condition or an accident/incident involving the resident.Assumes any duties that are assigned by the RN Supervisor/ADNS to provide optimal quality resident care.Performs other related duties and attends meetings as may be deemed necessary by Supervisor / Administrator and or Director.by Jobble
