Job Description:

MetroWest Medical Center, a leading regional healthcare system, seeks a highly skilled Social Worker to join its team. This role is responsible for facilitating care along a continuum, ensuring patients receive optimal health, access to care, and appropriate resource utilization.Key Responsibilities:

• Care Coordination: Collaborate with patients, families, and healthcare teams to develop and implement effective care plans.
• Transition Management: Identify and assess patients at risk for readmission, and develop strategies to prevent avoidable readmissions.
• Psycho-Social Assessment: Conduct comprehensive assessments to identify patients' needs and develop interventions to promote timely throughput and safe discharge.
• Education and Support: Provide education to patients, families, and caregivers to support post-acute needs and promote patient satisfaction.
• Clinical Leadership: Lead a population of patients by service line and/or lead the team, providing guidance and support to Tenet performance standards.Requirements:
  • Education: Master's of Social Work (MSW)
