Information $2,425 per week About The Position Job Title:
Registered Nurse (RN) Case Manager Job Specialty: Case Management
(Utilization Review) Job Duration: 13 weeks Shift: 8-hour days,
Monday to Friday, 8:00 AM - 4:30 PM or 8:30 AM - 5:00 PM Guaranteed
Hours: 40 hours per week Experience: Minimum of 2 years License:
Will consider without specific state license; exceptions apply
Certifications: Basic Life Support (BLS) Must-Have: - Familiarity
with Medicare, Medicaid, managed care, and other third-party payer
review requirements Job Description: - The RN Case Manager follows
patients, plans discharges, and collaborates with multidisciplinary
team members. - Works in coordination with Utilization Review, with
knowledge but no direct responsibility for it. - Functions as part
of a small case management team with potential assignments on
various inpatient units, including Med/Surg, ICU, Telemetry, and
Stepdown. - Assists with Special Care Nursery and PACU needs; no
behavioral health assignments. - Ensures medical necessity of
hospitalization and treatment per hospital and payer guidelines. -
Ensures appropriate utilization of hospital resources and
collaborates on cost-effective and efficient care management. -
Responsible for integration, coordination, and advocacy for
patients and families requiring post-hospital services. - Achieves
planned care outcomes by planning and arranging discharge services.
- Acts as a liaison with medical staff, other departments,
patients, families, and community resources to facilitate
utilization management and discharge planning processes. -
Familiarity with charting systems such as EPIC. Shift Notes: Monday
