practice environment.POSITION SUMMARY:The Program Manager is
responsible for operational activities of HRSA funded, RYAN WHITE
PART D program. The goals of the RYAN WHITE Part D include
monitoring the transition process for adolescents/young adults to
engage in care in the adult HIV program; enhancement of the
transition of pregnant women living with HIV/AIDS to the adults'
program after delivery; retention of adolescents, young adults, and
newly identified women in care; and monitoring quality of care for
HIV infected children, youth and women. The Program Manager
provides training, guidance and oversight to assigned staff.The
Program Manager will also perform medical case management and be
responsible for carrying out the goals of the HRSA Ryan Project
with regards to Women Living with HIV/AIDS. Major targets
include:
- Engagement of pregnant women living with HIV/AIDS into care,
and addressing barriers to care such as health insurance enrollment
and transportation.
- Engagement of targeted women living with HIV/AIDS at risk of
falling out of care or at risk of incomplete immune reconstitution
(CD4
- Transition of HIV infected pregnant women into HIV care in the
Adult HIV Program following delivery.JOB RESPONSIBILITIESPROGRAM
SUPPORT:
- Develops and implements methods for engaging and retaining HIV
infected patients, will develop and implement methods for ensuring
that quality measures are monitored and QI program is implemented.
Works within the guidelines of the HRSA contract, communicates as
needed with HRSA, and completes required reports to HRSA.
- Implements and monitors QI program, developing appropriate
programs to address areas that fall short of target goal.
- Develops and implements methods for engaging and retaining HIV
infected patients. Works within the guidelines of the HRSA
contract, communicates as needed with Program Officer, and
completes required reports to HRSA for program coordinator.
- Implements and monitors QI program, develop appropriate

goal.MANAGEMENT:
- Oversees the reporting of funded staff and works very closely
with Data Analyst to assure timely reporting
- Focuses efforts on transition program for youth to adults HIV
services. Provides necessary ongoing training and technical support
to ensure projects are responsive to Clinical Directors.
- Assists in training, supervision, coordination and performance
of staff as well appropriate. Coordinates with Associate Director
to track Personnel Action Forms (PAFs), performance reviews; works
with Program Director and Division administrators to address
performance issues.
- Prepares grant reports on quarterly and annual
basisADMINISTRATIVE:
- Oversees data management system ensuring timely and accurate
data collection. Compiles quantitative and qualitative data monthly
and uses data to determine status of program activities.
- Writes the quarterly progress reports and annual progress
reports for submission to all external funding
agencies/sponsors.
- Works in conjunction with Division administration to review
budget and identify program needs.
- Schedules meetings, coordinates audio-visual and technology
requirements, orders food/beverages, etc., as needed.JOB
REQUIREMENTSEDUCATION:
- Bachelor's Degree required (health studies, public health,
human services, social work, research, or related field
preferred).
- MASTER'S DEGREE PREFERRED (MSW, MPH)EXPERIENCE:
- Experience working in maternal-child health and or public
health program administration a plus
- Experience working with HIV+ clients and related issues,
including co-occurring disorders preferred.KNOWLEDGE AND SKILLS
- Ability to provide leadership and coordinate the work of a team
of assigned team members.
- Excellent oral and written communication skills; ability to be
detailed oriented in all notes and documentation.
- Ability to analyze needs and apply resources effectively to
meet those needs.
- Proficiency with MS Office (Word, Excel, Outlook), Internet and
web page usage/updating, social networking and other technologies
that can be used to further carry the program's mission.
- Demonstrated skills in the provision of services in a
multicultural environment.
- Bilingual skills appropriate to the patient population served
