Initiates and ensures the quality of pharmacy and drug distribution
services in all patient care areas. During their shift, assumes
direct responsibility of medication distribution, IV admixtures and
the safety & efficacy of prescribed drug therapy in accordance with
laws and regulations governing pharmacists and the practice of
pharmacy by the appropriate state and federal agencies. Provides
drug information to medical staff and patients upon request or when
the need is apparent. Routinely takes on added responsibility for
assigned area, function, or specialty.

Description:Essential Duties & Responsibilities: including but not
limited to:1) - - -Clinically reviews and assesses all medication
orders to ensure that medications entered into patient profiles are
safe for administration. -Assessment should include appropriate
drug selection, medication allergy, and dose, route of
administration, diagnosis, relevant laboratory results, therapeutic
duplication, drug interactions and age of patient.2) - - -Performs
assigned work during shift in an efficient and timely manner. -3) -
- -Practices in accordance with the state and federal laws, rules,
and regulations, including but not limited to Massachusetts Board
of Registration in Pharmacy, Department of Public Health, the Food
and Drug Administration, and the Drug Enforcement Administration.
Maintains certification and credentials in accordance with the
Board of Registration and adheres to institutional policies and
procedures.4) - - -Reviews and verifies medication orders with
consideration, including but not limited to the following patient
specific parameters: diagnosis, weight/body surface area, age,
allergies, organ function, laboratory values, medication
duplication, drug interactions. Clarifies orders deemed to be
inappropriate, incomplete, or otherwise unclear with the prescriber
in a timely manner.5) - - -Documents and communicates pertinent
information and discussion with the healthcare team and pharmacy
staff to ensure continuity of care.6) - - -Provides drug
information to physicians, nurses and other health care
professionals.7) - - -Checks and verifies sterile and non-sterile
medications.8) - - -Supervises the activities of students,
technicians, and residents within the pharmacist area of
practice.9) - - -Promotes a culture of patient safety by submitting
medication related events and near misses.10) - - -Completes daily
pharmacy surveillance tool monitoring, including follow-up and
documentation, as well as other daily patient care initiatives and
relevant reports.11) - - -Works collaboratively with all
pharmacists, pharmacy technicians, and pharmacy personnel. Upholds
a culture of respect and teamwork across the Pharmacy
Department.12) - - -Precepts pharmacy students, resident rotations,
and resident-led quality improvement projects. May serve as
Residency Program Coordinator or Residency Program Director. Serves
as a role model and training mentor for other staff.13) - -
-Demonstrates competency for Clinical Pharmacist I, Clinical
Pharmacist II and in all practice areas in which the Pharmacist's
duties are performed. -Has completed all required departmental core
competencies for areas of practice and specialty as applicable.14)
- - -Actively participates in continuous performance improvement
activities of the department, including providing suggestions on
how to improve day-to-day operations15) - - -Regularly participates
in department initiatives including but not limited to supporting
training & education, quality, medication stewardship, presenting
requests at Pharmacy & Therapeutics Committee, creating and
maintaining policies and procedures, conducting assessments of
workflows, and/or developing departmental tools and resources.16) -
- -Supports a specific core area of practice through
interdisciplinary education, ongoing specialty training and as a
resource for Pharmacy staff.17) - - -Develops and updates clinical
guidelines, policies, procedures and presents hospital wide
educational initiatives.18) - - -Serves on hospital
interdisciplinary committees or workgroups as needed to improve
process and quality outcomes.19) - - -Supports IRB-approved
research activities, medication use evaluations, or cost-savings
initiatives.20) - - -Incorporates Lahey Hospital and Medical Center
Guiding Principles, Mission Statement and Goals into daily
activities. -21) - - -Complies with all LHMC Policies.22) - -
-Complies with behavioral expectations of the department and
LHMC.23) - - -Maintains courteous and effective interactions with
colleagues and patients.24) - - -Demonstrates an understanding of

-Participates in and successfully completes Mandatory Education.28)
- - -Performs all other duties as needed or directed to meet the
needs of the department.Minimum Qualifications:Education: -Must be
a graduate of an accredited college of pharmacy with a current MA
registration in Pharmacy.Licensure, Certification, Registration: -
Must be eligible for a Massachusetts pharmacist license, advanced
certification (e.g. BPS) in applicable area within 2 years of
hire.Skills, Knowledge & Abilities: -Requires a working knowledge
of all basic aspects of health-system pharmacy (e.g. drug
therapeutics, sterile compounding, etc.)Experience: - -PGY 1 + 1-3
years or PGY 1 + PYG 2 or equivalentAs a health care organization,
