Job Description:
A company is looking for an ED Safe Discharge RN to monitor
real-time notifications and provide discharge coordination for
optimal transitions of care.
Key Responsibilities:
Collaborate with ED and outpatient care teams to create
individualized transition plans for members
Serve as a clinical liaison with hospital staff and perform care
coordination for ED discharges
Conduct outreach following ED discharge to confirm access and
completion of discharge plans
Required Qualifications:
Active RN license in current state of residence
4+ years of nursing experience
2+ years of Emergency Department experience
Knowledge of utilization management and discharge planning
Proficiency in Microsoft Office applications
